Authors: RusseLL, D., Rosenbaum, P., Gowland, C., Hardy, S., Lane, M., Plews, N., McGavin, H., Cadman, D., Jarvis, S.  Age-Range: Children
"The GMFM is a clinical measure designed to evaluate change in gross motor function in children with cerebral palsy. There are two versions of the GMFM: the original 88-item measure (GMFM-88) and the more recent 66-item GMFM (GMFM-66). Items on the GMFM-88 span the spectrum from activities in lying and rolling up to walking, running and jumping skills. The GMFM-66 is comprised of a subset of the 88 items that has been shown to be unidimensional." (From the link above)

Ages & Stages Questionnaires - Second Edition (ASQ)

Screening tool for developmental delay

Authors: Bricker, D., Squires J.,  Age-Range: Birth 5.0 years
"Parents complete the simple, illustrated 30-item questionnaires at designated intervals, assessing children in their natural environments to ensure valid results. Each questionnaire can be completed in just 10-15 minutes and covers five key developmental areas: communication, gross motor, fine motor, problem solving, and personal-social. Professionals convert parents' responses of yes, sometimes, and not yet — in just 2-3 minutes — to color-coded scoring sheets, enabling them to quickly determine a child's progress in each developmental area" (Quoted from Publisher) Takes about 10-15 minutes. Published in English, Spanish, French and Korean, other translations are available for licensing.
